We’re the company with heart

In-Home Care Coordination

When home is where you want to be, we help make it possible. Our in-home care coordination provides one-on-one support so seniors can remain independent, safe, and comfortable in familiar surroundings.

Care Coordination Services

We provide care coordination services for individuals who choose to remain in the comfort of their own homes. While bringing care into the home is important, we go further, our focus is on creating consistent, personalized support that preserves and enhances each client’s independence.  Our services are different then a traditional caregiving agency.  We work to avoid the common drawbacks of bringing care into the home, which may include – privacy invasion, dependable care, compatibility of caregivers, clear guidelines and job expectations for your home as a workplace.

To ensure your life routines and household norms are honored, we develop a customized Coordinated Care Binder, complete with secure online access. This becomes your home’s go-to manual, capturing the details that matter most, including in-home safety protocols. With your input, we build a Team Playbook for caregivers, care providers, and family care partners, guiding how everyone interacts, collaborates and appropriately responds.

Our services include establishing your home safety planemergency and disaster plan, and daily household routines so that every member of your support team, whether near or far, understands their role and how to help effectively.

Once your preparedness protocols are in place, you can enjoy the peace of mind that comes from knowing there is a clear, coordinated plan, for both safety and daily care, ready for whatever comes next.
